If the membership application is approved, I shall abide by the Memorandum and Articles of Association of the CSKOBA. These documents are publicized on https://www.cskoba.org
The personal data provided in this form will be retained by the CSKOBA, which will be used by the CSKOBA to facilitate communication between the CSKOBA and/or Chan Sui Ki (La Salle) College and yourself, or in connection with activities organized, sponsored or approved by the CSKOBA and/or Chan Sui Ki (La Salle) College.
I have the right to:
1. obtain a copy of my personal data kept by the CSKOBA by writing;
2. request that necessary corrections be made if such data are inaccurate; and
3. have my personal data removed from the communication of the CSKOBA and/or Chan Sui Ki (La Salle) College.
4. terminate my CSKOBA membership by providing a written request to the CSKOBA.
